Transitioning to the NDIS

We have a four-step process that will make sure that you get the best out of NDIS. Step 1 – Pre-planning

To start with, you need to draw up a plan. Think of your current support need and what your future support might be. You need to also get information from your GP, specialist and other allied health professionals to ascertain the level of support you will be needing. All this will enable you to access the right funding from NDIS.

Step 2 – Meeting with the NDIA

Having understood the level of support, you will request and be contacted by the National Disability Insurance Agency (NDIA) to set up a planning meeting. To create your first plan, you will meet with an NDIS representative or Local Area Coordinator to have a conversation about your current situation and supports. Bridges Care staff can be there in the meeting to assist you in developing your plan.

Step 3 – Starting your plan

Once your plan is approved, it is time to activate it. There are supports available to help you implement and monitor your plan, this is called coordination. Coordinators will link you with service providers, address any barriers to accessing services and build your capacity to gain more independence.

Step 4 – Turning your plan into action

Bridges Care has various ways to support you to implement your plan. First, we develop an implementation plan based on your goals and strength. Our implementation plan is participant-centred. In other words, you are in control of your choice, all we do is to support and guide you, using our extensive experienced staff in the disability services.
