Transitioning to the NDIS

Step 1 – Pre-planning

To start with, you need to draw up a plan. Think of your current support need and what your future support might be. You need to also get information from your GP, specialist, and other allied health professionals to ascertain the level of support you will be needing. Step 2 – Meeting with the NDIA

Having understood the level of support, you will request and be contacted by the National Disability Insurance Agency (NDIA) to set up a planning meeting. Bridges Care has various ways to support you to implement your plan. First, we develop an implementation plan based on your goals and strength. Our implementation plan is participant-centered.
