Transitioning to the NDIS

Step 1 – Pre-planning

To start with, you need to draw up a plan. Think of your current support need and what your future support might be. You need to also get information from your GP, specialist, and other allied health professionals to ascertain the level of support you will be needing. All this will enable you to access the right funding from NDIS.

Step 2 – Meeting with the NDIA

Having understood the level of support, you will request and be contacted by the National Disability Insurance Agency (NDIA) to set up a planning meeting. To create your first plan, you will meet with an NDIS representative or Local Area Coordinator to have a conversation about your current situation and support. Step 4 – Turning your plan into action

Bridges Care has various ways to support you to implement your plan. First, we develop an implementation plan based on your goals and strength. Our implementation plan is participant-centered. In other words, you are in control of your choice, all we do is support and guide you, using our extensive experienced staff in disability services.
